In checking the box below, I do hereby release Kohler Co., Sports Core, Yoga on the Lake, BOLD CYCLE and their respective agents and employees from any and all liability, demands, suits, actions, claims, or judgments of any nature, and costs and expenses including attorney’s fees incidental thereto, for any injury, damage, illness or death which I sustain during or as a result of my participation in the program or which is in any other way related to the program. I represent that I have consulted with my physician regarding any past or present injury, illness, or other condition which may affect my ability to participate in the program and that my physician has authorized and approved my participation. I acknowledge that this release is being relied on by the above persons in permitting me to participate, and that this release shall be binding on me, my heirs, assigns and personal representatives. I acknowledge that the above mentioned may use photographs of me with or without my name and for any lawful purpose including publicity, illustration, advertising and web content.
