In checking the box below, I acknowledge that I have carefully read this “waiver and release” and fully understand that it is a release of liability. I expressly agree to release and discharge Body Alive Corporate, LLC. and all of its affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I otherwise have to bring a legal action against Body Alive Corporate, LLC. for Personal injury or property damage. I am aware that my activities inside Body Alive are recorded via audio and video at all times and agree to that. I agree that Body Alive may use any video footage or photographs. I agree to pay for classes before class and understand that there will be no refunds. To ensure that all Body Alive members have access to all available classes, I agree to give at least 3 hours notice to cancel a class reservation, by emailing [email protected], calling the studio, or canceling via the Body Alive app or website. If I do not cancel before 3 hours from the start of class, I will be charged the $5 late cancellation fee. (12-hour policy for Cycle and Bounce Classes). An authorized Body Alive representative may, in her or his sole discretion, waive this policy in case of true illness, injury, or emergency. I understand that any introductory or promotional offer will auto-enroll into an Orange Membership. I agree to give 72-hour notification prior to my membership renewal to cancel and understand that there will be no refunds. I understand that all Orange Memberships are monthly auto-renewing charges and I have 72 hours prior to the renewal date to submit a written request of cancellation and refunds will not be issued.
