In checking the box below I agree that Hot Yoga Plus San Rafael, is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes at Hot Yoga Plus San Rafael may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Hot Yoga Plus San Rafael or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
In consideration of and as inducement to enrolling as a student of Hot Yoga Plus San Rafael LLC, I represent and agree as follows:
(1) I have been examined by a licensed physician within the past six months and have been found by such physician to be in good physical health and fully able to perform all yoga exercises which I am to learn and perform during my enrollment.
(2) I understand that this can be strenuous and I voluntarily participate with full knowledge of the risk of injury, property loss or even death. In addition, I will not hold Hot Yoga Plus San Rafael responsible for safe keeping my belongings.
(3) I agree that neither I, nor any of my heirs, will hold you, your partners, instructors, or employees responsible for any injuries suffered by me caused by negligence or otherwise. I understand and acknowledge that I am to receive instruction in Yoga theory and exercises only, and I will not hold you, your partners, instructors, or employees to any higher standard of care than that applicable to a school of yoga theory and exercises.

(4) I AGREE TO HAVE MY PHOTO TAKEN IN ORDER TO ATTEND CLASSES.
(5) I agree it is ok to charge my account/credit card for any outstanding balance on my account for rental items, retail items, drinks or anything items taken as account purchases. You are not allowed to make account purchases without a credit card on file.
(6) Outstanding classes will be charged to your credit card at the amount of 22.00 dollars per class if you have no current package.
