In checking the box below I agree that TransForm Colorado is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise. I further agree to the terms and conditions of the studio cancellation policy. I agree that all classes require an 12 hour cancellation in order to avoid late cancel and no show fees. I agree to pay a $15 late cancel fee and a $25 no show fee. I understand that if I am a ClassPass client and I no show for a class that I will be charged a fee from ClassPass AS WELL AS Transform.

 

Toe socks are required in all classes. If you do not bring them to class you will be required to purchase a pair. Client understands they will not be able to participate in class if they do not have toesox with them, and cancellation fees or loss of class may incur if client doesn't bring/buy them. A valid credit card is required on all accounts in order to participate in any of our classes. I agree and understand that any balances I may incur on my account that I will be required to pay in full before being allowed to participate in any class at Trasnform. I understand that if I am late my spot may/will be given away to a client waiting.
