Terms and Conditions
I acknowledge that the activities I am going to undertake are of a dangerous nature and by participating in them I am exposing
myself to certain risks. I acknowledge and understand that whilst participating in these activities there is the risk of being injured
both physically or mentally, possibly resulting in death. I understand that my personal property may be damaged or lost, that other
persons participating may cause me injury and/or may damage my property. I may also cause injury to other persons and/or
damage their property.
Conditions of activities being conducted may vary without warning. I assume the risk and responsibility for any injury, property or
death resulting from my participation in any activities.
I agree to release and indemnify Cycle Republic as follows:
- I participate in the activity at my own risk and responsibility. I release, indemnify and hold harmless Cycle Republic, its
servants and agents from and against all and any actions or claims that may be made by me or on my behalf by other
parties in respect to any injury, loss, damage or death caused to me or my property whether by negligence, breach of
contract or in any way whatsoever.
- I agree that in the event that I am injured or my property is damaged, I will bring no claim, legal or otherwise against Cycle
Republic in respect of that injury or damage.
- Before signing this document I have read and understood it and acknowledge that it affects my legal rights.
- I understand that this document is intended to provide my fitness instructor with accurate knowledge of any physical
conditions and/or limitations I may have to ensure that I am given a safe exercise program.
- I agree that I have answered all questions to the best of my knowledge and that I understand all of the risks involved in
participating in such physical activity.
