Studio Liability Release
In checking the box below, I agree that trampoLEAN is in no way responsible for the safekeeping of my personal belongings while I attend class. I assume all risk of loss for any of my personal belongings. I understand that classes at trampoLEAN are physically strenuous, and I voluntarily participate in them with full knowledge that there is risk of personal injury or death. I hereby assume all risk of loss for any personal injury or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against trampoLEAN, its members, officers or employees for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or for any other reason whatsoever.
- All purchases are non-transferable and non-refundable.
- Schedule classes online. Cancellations & rescheduling have a 24 hour policy.
- Return all equipment at the end of class.
- Do not put anything on a trampolines other than your body.
- Any injuries or limitations must be reported prior to each class.
- Towels are recommended during class.You are encouraged arrive at least 10 minutes prior to class.
By agreeing to these rules and regulations, I hereby acknowledge that I have voluntarily chosen to participate in a program of intense physical exercise. I further acknowledge that trampoLEAN strongly recommends that I consult with my physician prior to commencing any class. I acknowledge that I am fully informed of the strenuous nature of this program and the possibility of adverse physiological occurrences including but not limited to abnormal blood pressure, fainting, heart attack or death. By agreeing to this disclosure I assume all risks for my health and well being, and fully release and hold harmless for any responsibility, cost, or damages trampoLEAN , its Instructors, members and employees. I hereby acknowledge that I, and any minor under my care, have voluntarily chose to participate. I assume all risk for my health and well being and any minor under my care, and fully release trampoLEAN , it's officers, members and employees.
