In checking the box below I agree that clique fitness LLC is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes at clique fitness LLC may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.

Also in checking this box you agree to be at the age of 14 years or older and have guardians permission to attend. All Teens must have a guardian attend their first class to sign a waiver at the studio. 

I understand and agree that my card will be charged a $10 late fee if I'm a monthly member or a class credit if I am a pack or single class holder when late cancelling which is considered 6 hours or less before the class.
