I understand and agree that Your Destination Fit, their employees, sub-contractors, volunteers, sponsors, and affiliates, including any such location that services are performed are not responsible for any injury, illness or property damage arising out of the Your Destination Fit program, even if caused by their ordinary negligence.  I understand that participation in the Your Destination Fit program involves certain risks, including, but not limited to, serious injury.   I am voluntarily participating in the Your Destination Fit program with knowledge of the danger involved and agree to accept all risks.  I authorize Your Destination Fit and it's members to publicize media that contains photos/videos of myself.  I hereby waive and release myself, my family members, and my heirs, of any and all past, present, or future rights or claims against Your Destination Fit, their employees, sub-contractors, volunteers, sponsors, affiliates, or any facility, center, school, personal property or park that services are performed.
