I represent that I am in good health and have no disability, impairment, injury, disease, or ailment, preventing me from engaging in exercise, including, but not limited to spin, high intensity interval training, or any exercise which could cause increased risk of injury or adverse health consequences as a result of such exercise. I represent that there is no medical or physical condition that would preclude me from using 360 CYCLE STUDIO facilities and/or classes and further represent that I have not been instructed by any physician not to use a 360 CYCLE facility and/or classes or not to use any health club facility and/or classes.

I authorize the use of photographic or electronic reproductions of me for any purpose, including, but not limited to public media as may be deemed appropriate by SLEVEN SPORTS 360 CYCLE STUDIO. (I understand that I may be identifiable from such photographic or electronic reproduction).

By signing this form, I acknowledge that I have been offered &/or read the 360 CYCLE STUDIO's Liability, Privacy, Cancellation, Refund policies and Media Release and agree.
